On the back you will find a Pre-Surgical/Pre-Anaesthetic Testing form that we would like you to show to your owner. Please have them read and complete this form and bring it with them when they drop you off at the clinic. Pre-anaesthetic bloodwork gives the doctor an inside look at some of your vital internal organs. We are especially concerned with the health of your liver and kidneys as these organs help the body get rid of the medications used during anesthesia. We strongly recommend completing these blood tests to help ensure your safety.

***PRE-SURGERY CHECKLIST***
  No food for you after 9:00 p.m. the night before. This means no sneaking into the fridge for a midnight snack!! The only thing that you can have is water.
  No water after you get up in the morning. (Your owners may have a coffee though!!)
  A bathroom run before you hop in the car!
  Have your owners complete and sign the Pre-Anaesthetic Testing form and bring it along with them when they drop you off. Make sure we have a phone number where we can reach your owner during the day in case we have any questions.
  If you are a dog, have your owners bring in a sample of your urine so that we can test your kidney function. Cost is only $10 if you've supplied the sample. Remember to go easy on your owner while they chase you around the yard!! A stool sample is also recommended to make sure you're free of intestinal parasites! (Cost is $26)
  Relax!! The anaesthetic we use is the safest available and we will keep a close eye on you the entire time you are at the clinic.
You can phone the clinic at around 2:00 p.m. to see how you are doing and to find out when you can go home!! We usually send patients home around 4:00 p.m. but we are open until 5:00 p.m (6:00 p.m. on Tuesday and Wednesdays). Okay, that's it! We look forward to seeing you soon. Please have your owners call if they have any questions that you can't answer.


 

 


PRE-ANAESTHETIC WELLNESS TESTING CONSENT FORM

At Central Valley Veterinary Hospital, it is our purpose to treat your pets like family members and to educate you on how to best take care of them. In keeping with this purpose, we strive to make the general anaesthetic your pet is about to undergo as safe and comfortable as possible. Every animal will receive a complimentary physical examination by the veterinarian to rule out any readily detectable abnormalities.
In addition, we strongly recommend the following laboratory tests in order to detect any internal conditions which may increase your pet's surgery or anaesthetic risk or which may be aggravated by the anaesthetic. The added benefits of performing the tests are that they will serve as reference values for future use should your pet become ill. Test results will be available immediately as they are performed on site using the latest technology.

LASER SURGERY CONSENT FORM
Advances in veterinary medicine and LASER surgery now make LASER use in our surgical procedures available to our clients. We continually strive to provide the best possible care for your pet. We feel that LASER surgery provides the best possible care for your pet. The main reasons for this are as follows:
1. The LASER produces little or no blood loss for your pet because the LASER seals the small blood vessels
2. The LASER produces little or no swelling for your pet because the LASER seals the lymphatic vessels.
3. The LASER produces less post operative pain for your pet because the LASER seals the nerve endings.
4. The LASER allows precision in removal of tissues.
5. The LASER may allow fewer stitches or eliminate sutures completely.
I understand that LASER surgery is an option* and I have been advised of the advantages this procedure will offer.
  *The use of the LASER for routine, elective surgeries is an option.
*The use of the LASER in complex surgical procedures may be required.

How Do We Deal With "Complications"?

.....The subject of who pays for the costs involved in treatment of complications after surgery or dentistry or even medical treatment can be an awkward one because complications are usually unexpected and unpredictable. We usually all like to think that everything will progress perfectly smoothly and patients will respond as expected but this is not always the case. Many factors can be involved, some of them beyond any of our control. Sometimes, the problem lies with the patient (ie: a pet that licks compulsively at an incision or reacts adversely to a medication). Other times, the owner forgets some crucial instruction. Occasionally, the veterinarian or staff may perform a procedure in a certain way that, in retrospect, may have been better done in another way.

.....At Central Valley Veterinary Hospital, we take the approach that everybody is imperfect, but we all have the best intentions. We all want a happy treatment outcome for the pet. We look at the veterinary-client-patient relationship as a partnership where we all bear equal responsibility. Consequently, when it comes to who pays for complications, we believe in sharing the expense equally. We pay half and you, the pet owner, pay the other half. In other words, we bill as usual and discount the total by 50%. Hopefully, this is satisfactory.
